"Dear Patients, good day and I hope you and your family are safe. Due to the current coronavirus threat and restrictions to travel, I made this service available so I can continue serving my patients. With this, there are some reminders regarding this telemedicine service:

1. Patients will be paying prior to the consult to be able to secure the time slot. This is to avoid "no show" patients who will deprive other patients the opportunity to be seen.

2. The payment for the consult booking is non-refundable. Hence, please log-in to the video call during your scheduled consult.

3. For HMO patients (Maxicare and Medicard only), please secure an LOA prior. Medicard patients can send a copy of the LOA to my secretary. For Maxicare patients, Maxicare will be sending a copy of the LOA directly to me.

4. A link to the video call will be sent to you via email and SMS. My assistant will likewise send an SMS reminder to you the night prior to your day of consult.

5. If there are some changes in your schedule, you may revise your booking until 5pm of the day prior to your scheduled consult.

6. Your medical records will be kept in this Clinicko EMR system, a HIPAA compliant platform with data encryption.

7. Please refrain from recording the conversation. Recording the consult in any form without the consent of the doctor is prohibited by law.

8. There are some innate limitations with telemedicine. Inability to physically examine the patient and internet connection difficulties are some examples. Please bear with us. Rest assured, I will do my best to offer the best possible care given these limitations. However, arrangements will be made to see you in person should our clinical assessment demand this.
